Anyone wishing to be screened at the Women’s Heart Health Day event can save time by downloading, printing and completing our required consent and survey form before the event.  Click on the link below, select the language of choice and make sure you set your printer to 8.5 X 14” to print it on legal size paper. Consents printed on 8.5 X 11” paper cannot be read by our scanner and will not be accepted.
